
New Requirement for Summary of Site Condition with Submissions for Ministry Review 11/30/2007

Effective July 1, 2007, the requirement for a Summary of Site Condition (SoSC) replaced Protocol 5 information requirements for all service applications submitted via the Approved Professional process.
The ministry now requests that applicants include a completed SoSC form with all service applications submitted for contaminated sites legal instruments and other services outlined in section 7.1 of the Contaminated Sites Regulation. Note that fees for review of the SoSC are not currently required for applications submitted for ministry review.

Requirements for service applications requesting contaminated sites legal instruments and for ministry review include: * A completed Contaminated Sites Services Application Form * A completed Summary of Site Condition (SoSC) (electronic and hard copy) * The site investigation checklists in Technical Guidance 10 and 11 * A complete package of the reports and plans relevant to the instrument requested * A cheque for the full amount of fees payable to the Minister of Finance
Please note that if a section in the SoSC form is not applicable you should enter "n/a" in that section so the ministry will know that the section has been considered. Also note that Section 8.2 should not be completed by those submitting applications for ministry review.
